Does a 12,000-lb box truck working only around Albany need driver qualification files?

Yes. New York's commercial motor vehicle threshold for in-state work is the same 10,001 lbs as the federal rule (17 NYCRR § 820.1(c)), so the driver needs a qualification file even if the truck never leaves the Capital Region.

Is my long-time Albany driver exempt from the medical certificate?

Possibly, for in-state work only. An intrastate CDL driver first licensed before September 9, 1999, or a non-CDL driver first licensed before January 1, 2004, is not required to have the medical exam (17 NYCRR § 820.3(d), (f)). The exemption ends the moment the driver takes a load into Massachusetts or Vermont.

Can I hire an 18-year-old driver for my Albany company?

For intrastate routes, yes: in Class B and non-CDL vehicles under 17 NYCRR § 820.3(g), and in Class A tractors if the driver holds an intrastate-only Class A CDL from the DMV's young adult program. A run east on I-90 into Massachusetts requires a driver who is at least 21, outside federal pilot programs.
